Job Description:
JOB SUMMARY:
As a valued member of the team, the Social Worker provides support and assessment services to the residents and families / substitute decision maker (SDM).
The Social Worker will assist in emotional and financial issues or concerns of the resident. The Social Worker shall conduct their practice in alignment with the organization’s Mission, Vision, and Values. They are also responsible for ensuring resident wellbeing and safety. The Social Worker is an integral member of the interdisciplinary team.
KEY RESPONSIBILTIES:
- May assist with resident move-in / move-out paperwork, transfers and tour potential residents and family members as required.
- Co-ordinates and / or facilities / participates in interdisciplinary resident care conferences.
- Arranges family meetings as per guidelines and as required in consultation with the ED / DOC / Clinical Lead.
- Facilitates and participates in a support group for residents and families.
- Co-ordinates and facilitates Ethics Committee meetings and completes minutes.
- Completes assessments as required (I.e. MMSE, competency assessment) and/or willingness to learn how to complete assessments.
- Liaises with residents, families, outside agencies and community resources. Provides one to one support for residents and families.
- Supervises Social Work students’ placements as per management approval.
- Document all resident specific information on PCC; Maintains appropriate records.
- Review the 24HR report each shift to be kept up to date on resident’s progress and determine where support is needed. Recommended areas of focus: family note, move-in/move-outs, heath status, incident reports and palliative care.
- Conduct regular walkabouts to each neighbourhood.
- Process referrals as required (verbal, email and written, via referral forms).
- Attend to issues that relate to PG&T if a resident need something, gain approval from PG&T and ED and purchase items.
- Assists in providing support in emergency situations, such as; suicide risks – support and make appropriate referrals as needed and participate in care planning.
- Advanced care planning – (i.e. MOST, POA, Enduring POA, RA7, or 9, funeral home arrangements).
- Participates in educational opportunities.
- Adapts and adjusts according to the changing needs of the resident population.

Required Skills:
QUALIFICATIONS:
- Experience working with seniors, particularly in a long term care setting.
- Knowledge of community resources.
- An energetic individual that possess effective leadership, organizational, and interpersonal skills.
- Demonstrate initiative, the ability to work independently and/or in a team setting.
- Innovative resident-centered approach to Social Work.
- Previous experience in a LTC setting as a Resident Social Worker.
- Competency in MS Office, and proficiency in using computers / computer programming.
- Effective verbal and written English communication skills required.
EDUCATION:
- Post-secondary diploma or degree from an accredited School of Social Work and / or related field
- Current enrollment in a community college or university in a diploma or degree program in such a field
- Current Standard and/or Emergency First Aid and CPR Certification
- Membership in the ON Association of Social Workers (OASW)
- Experience working with seniors, particularly in a long term care setting.
