Client Care Coordinator /RPN - Baawaating Family Health Team
Full-Time Permanent

We work with our 11 First Nation community partners and the Urban Indigenous population in Sault Ste. Marie in the provision of primary health care, traditional healing, mental wellness and addictions, home and community support services and health promotion services. We have been approved as the first Indigenous led Ontario Health Team in the province by the Ministry of Health.

The opportunity: The purpose of this position is to provide clerical and clinical support to the Baawaating Family Health Team and to coordinate client focused care to community members seeking and accessing health care services. The Client Care Coordinator/RPN is the first point of contact with clients and will play a critical role in the efficiency and flow of the clinic. The scope of this position is to work collaboratively as part of a diverse health care team and focus on coordinated and quality care for clients.
The position requires the Client Care Coordinator/RPN to perform duties related to managing the electronic booking of appointments and follow up, monitoring of referrals, consult notes, labs and other correspondence required in the care of clients.
The incumbent will ensure quality data entry into the electronic medical record and will be required to participate in quality improvement initiatives as part of the primary health care team.

This position will apply academic training and experience within the health clinic setting by the incumbent assisting and working closely with front-line health care professionals. Advocating on behalf of clients and helping clients navigate the health care system as they transition between health services both internally and externally will be an integral and critical role within this position.

Required Skills:
What you need to bring with you:
- A college diploma in the field of a Registered Practical Nurse (RPN)
- Registration in good standing with the College of Nurses of Ontario and Registered Practical Nurse Association
- Duties specific to the RPN role will include taking/recording vital signs, assisting/administering injections, collecting samples for lab testing as assigned, perform basic foot/nail care, and other related duties
- Respect for sensitivity towards, as well as knowledge and understanding of Anishnabek culture and language and the Seven Grandfather Teachings
- Must possess excellent oral and written communication and interpersonal skills
- Excellent organizational skills and ability to promote teamwork
- Must be self-directed, resourceful and possess critical thinking skills
- Strong knowledge and proficient in the use of computers and various software applications, such as Word, Excel, Publisher, Outlook
- Experience in the use of electronic health records would be a definite asset
- A clear criminal reference check is a condition of employment
- Must possess a valid driver's license and access to a reliable vehicle

What to expect? Given the traditional practices of Indigenous people, from time-to-time exposure to smoke from the burning of sacred medicines such as tobacco, sweet grass, sage, or cedar may occur.
