Position Status Temporary Full-Time and Permanent Part-time
Salary $30.77-37.58/hour, per Local 684 Collective Agreement
PURPOSE OF POSITION
The Housing Case Manager is responsible for the delivery of case management services to individuals experiencing mental illness disorders, intellectual disabilities and/or substance use disorders by assisting clients in connecting them to resources that can considerably improve the quality of their lives and increase their overall wellbeing. The position is community-based, requiring Housing Case Managers to meet clients within their environments. The Housing Case Manager is responsible for assessing clients' needs and support systems; planning for crises and
helping clients develop coping mechanisms; explaining these scenarios and their attendant concerns with compassion; and coordinating and monitoring of the client's use of these services. The Housing Case Manager carries out these responsibilities in accordance with the mission, vision and values of CMHA Algoma, relevant legislation and regulations and organizational policies, to promote the delivery of efficient and high-quality
services.
KEY RESPONSIBILITIES
Client Identification and Eligibility for Services, Assessment, Planning, Implementation, Evaluation, Transition, Community Relations and Advocacy, and General Organizational Responsibilities
- Delivers case management and housing support to individuals experiencing intellectual disabilities, mental illness disorders and/or substance use disorders. These individuals are either homeless or at risk of becoming homeless.
- Delivers support services in coordination with supportive housing rent supplement units.
- Duties vary based on assigned responsibilities in each of the following areas: Addiction Supportive Housing (ASH), Mental Health & Justice Supportive Housing (MHJSH)

Required Skills:
Education, Certification & Experience
- A university degree in a Human Services related field
- Current registration with OCSWSSW or a regulated body under the Regulated Health Professions Act, if eligible
- Demonstrated knowledge of community resources and organizations
- Certification in ASIST and First Aid/CPR (or willing to obtain on own within 3 months)
- Minimum three years experience in the fields of mental health and addictions with an emphasis in community-based services
- Knowledge and experience in conducting bio-psychosocial assessments
- Experience in the criminal justice system and knowledge of community resources
Knowledge, Skills and Abilities
- Ability to work in partnership with clients
- Extensive knowledge and understanding of the mental health and addictions services
- Extensive knowledge of standardized screening and assessment tools
- Extensive knowledge of community services, supports and resources
- Ability to apply Recovery principles and empowerment-orientated philosophies and practices in work with clients
- Demonstrated initiative, dependability, creativity, and attention to quality of work skills and abilities
- Ability to demonstrate diplomacy and negotiation skills in client and system advocacy
- Ability to make sound clinical judgments
- Excellent organizational and time management skills
- Excellent interpersonal skills
- Excellent written and verbal communication skills
- Ability and flexibility to work independently and as part of a team
- Excellent computer skills and competency in software applications used by the agency
- Demonstrated ability to engage in reasonable self-care strategies that incorporate balancing work/life responsibilities
- Excellent problem-solving and decision-making skills

Requirements:
- Acceptable Police Vulnerable Sector Check upon hire and to be maintained throughout employment
- Ability to provide effective client services in English and French, is preferred
- Able to meet the essential physical requirements of the position including ability to sit for prolonged periods
- Valid driver's license and access to a reliable vehicle
