Job Description:
Position: Registered Practical Nurse
Hourly Range: ALIGN WITH GRFN PAYBANDS
Location: Wellness Centre
Duration: Full Time

Reporting to the Home & Community Care Supervisor, the RPN uses her/his nursing skills to effectively assist H&C.C clients to attain their optimum level of independent functioning in activities of daily living.

Duties
- Providing case management services;
- Participate in case review conferences;
- Continually assessing the clients health status;
- Consulting with the clients physical and other health agencies/ professionals;
- Maintaining client records;
- Maintaining Client confidentiality according to PHIPPA and governing body;
- Providing Support to the client/Family by acting as a resource/ advocate;
- Providing teaching and education to client and caregivers;
- Maintaining current skills through;
- Attendance at workshops, in-services and staff meetings
- Reading/ viewing applicable education materials
- Recertification's
- Document the results of the assigned tasks;
- Involve client/ family in performing tasks, whenever possible;
- Recommends to the HCC RN, revisions to the care plan as appropriate;
- Phlebotomy considered an asset;
- Foot Care experience considered an asset and/or willing to become certified within 6 months of hire;
- Performing other duties as may be required or assigned

Required Skills:
- Must be a Registered Practical Nurse, currently registered and in good standing with the CNO;
- Required Degree or Diploma in Nursing from a recognized Post Secondary Institution;
- Previous Home and Community Care Health Nursing Experience in a First Nations Community is an asset;
- Previous experience working in a First Nations Community is an asset;
- Excellent oral and written communication skills;
- Ability to use good judgement in assessing difficult situations;
- Flexibility to keep pace with an ever-changing environment;
- Ability to be consistent and display a positive/helpful attitude;
- Willingness to accept changes in work practices and technology;
- Ability to work independently and collaboratively as a member of a team;
- Excellent organizational skills, time management skills, interpersonal and communications skills, both oral and written;
- Valid Ontario Drivers License;
- Must undergo a criminal record check prior to employment (At Applicant Expense)
