Job Description:
The Family Support Worker (FSW) is a member of the Healthy Growth and Development team which is responsible for the delivery of the Healthy Babies Healthy Children Program as mandated by the Ontario Public Health Program Standards 2018. Under the direction of the Program Manager, and in collaboration with the Public Health Nurse, the FSW works with families to develop parenting skills and knowledge, assist families with accessing community resources and services, and work to meet the identified family goals.

Required Skills:
Qualifications, Ability, Experience, and Skills for this position include (but are not limited to):

- Minimum grade 12 diploma;
- Demonstrates knowledge of strategies that support families in developing positive parenting skills;
- Experience working with families with young children an asset;
- Extensive knowledge and the ability to navigate and link clients to community resources and organizations;
- Ability to collaborate and communicate effectively with diverse individuals, families, and health and social services professionals;
- Ability to effectively work independently or as part of a multidisciplinary team;
- Use of a reliable automobile, valid \"G\" Ontario Driver's Licence and ability to travel is a requirement;
- Strong computer skills and an ability to learn new computer programs are essential;
- The ability to communicate (verbally & in writing) in French or any other language(s) will be considered an asset;
- Strong interpersonal, organizational and problem-solving skills;
- Must be prompt, reliable and maintain a good attendance record;
- Ability to work outside regular business hours, including weekends and evenings, as required;
- As a condition of hire, you are required to be fully vaccinated with a COVID-19 vaccine series, per our COVID-19 Immunization policy, unless medically contraindicated. Should you be the successful candidate, you will be required to comply with our COVID-19 Immunization policy that is in effect;
- A current Police Vulnerable Sector Check (PVSC) with satisfactory clearance is a condition of employment, at your own expense.
