Job Title: Adult Day Program Community Support Worker
Job Status: Part Time Maternity Leave 21 hrs./week
Starting Wage: $21.00/hr.
Reports To: Manager, Clinical Services
Hours of Work: 8:30 to 4:30. Mostly weekdays but weekends and evenings may be required.
Job Location: Blind River
Job Description:
- Working as an integral member of the Alzheimer Society Adult Day Program, the Adult Day Program Community Support Worker will work at supporting a variety of needs for clients attending the Adult Day Program in Blind River. The ideal candidate must be highly motivated, with a passion for persons living with dementia.
- The Adult Day Community Support Worker will provide a variety of activities in the community 2 days per week in Blind River and surrounding area.
- The Adult Day Program Community Support Worker will provide care in a safe and competent manner within their scope of practice.
Responsibilities:
- Plan and organize various activities for clients in an Adult Day Program setting 1 day per week.
- Monitor participants throughout the day, assist, participate with bathroom routines when required and support mobility needs.
- Monitor and provide proactive follow-up for clients and care partners to ensure ongoing collaboration across services and identify opportunities for new or emerging care options to meet changing needs and to address service gaps.
- Calmly and professionally react to crisis situations.
- Provide activation to individual clients in community 2 days per week.
- Conduct programs with clients on a regular basis.
- Provide support to co-workers, students and volunteers.
- Evaluate the effectiveness of the programming for the clients.
- Provide education and support to Persons Living with Dementia and their families.
- Act as a resource for inquiries from the general public regarding recreation/activation and Adult Day Program.
- Demonstrated knowledge and best practice in dementia, delirium, mental health disorders and their effect on persons, families, caregivers and the system of care.
- Ensure that client consents, privacy and confidentiality are maintained in compliance with legislation, professional standards/regulations and internal policies.
- Excellent communication (verbal and written).
- Ability to take initiative and be resourceful.
- Excellent problem solving and change management skills.
- Adhere to program standards and Alzheimer Society policies and procedures.
- Assume responsibility for personal upgrading in knowledge of Alzheimer's disease, and ongoing professional development.
- Participate in fundraising and other team development activities.
- Assume other functions as may be assigned by the Executive Director/Team Lead.
- Bilingualism (English/French) is an asset.

Required Skills:
Knowledge/Skills/Abilities:
- Post-secondary degree or diploma in Recreation Therapy or graduate of an approved Personal Support worker program. Other health disciplines may be considered given appropriate combination of education and experience.
- Knowledge of Alzheimer's disease and related dementias (ADRD).
- Experience working with persons with dementia and their families.
- Experience designing recreational programs to support individual needs as well as facilitating group activities.
- Knowledge of community health care agencies.
- Current Standard First Aid/CPR certification.
- Ability to work cooperatively with volunteers, staff, and community partners.
- Competency with Microsoft Office applications.
- Valid driver's license, regular access to a vehicle, and verification that the vehicle is insured for work purposes.
