Home Care Clinical Coach
Salary Range: $78, 570 to $96,631
Location: Head office in Cutler or office in Sault Ste. Marie (location dependent on where successful candidate resides)

Who you are? You are highly motivated to support the ongoing development of an Indigenous community-based home care service within the Maamwesying Ontario Health Team territory. Able to co-create the regional home care modernization implementation plan with Ontario Health, including required measurement and reporting. Have the ability to foster regional collaboration and contribute to broader system integration and ensure alignment of community implementation with the guidance of the Canadian Centre for Accreditation standards. Possess the skills to lead team engagements and teambuilding initiatives. Able to build competency amongst front-line community providers in the delivery of primary-level home care. Relevant experience to guide the home and community care quality improvement (QI) and practice change initiatives and consult on clinical care.

We work with our 11 First Nation community partners in the provision of primary health care, traditional healing, mental wellness and addictions, home and community support services and health promotion services. We have recently been approved as the first Indigenous lead Ontario Health Team by the Ministry of Health.

The opportunity: Maamwesying North Shore Community Health Services provides primary health, traditional healing, mental health, home and community support services and health promotion services to community members of the seven First Nations of the Mamaweswen North Shore Tribal Council, is inviting applications for the Home Care Clinical Coach / Supervisor to join the Home and Community Support Services Team. The Clinical Coach is responsible for working with partner First Nation communities and organizations to support operationalizing the Service Provider Organization services. The Clinical Coach will drive practice change, with a focus on improving access to and quality of home care, aligned to the provincial home care modernization plan. The Clinical Coach reports directly to the Director of Home and Community Support Services and closely liaising with the OH Northeast Home and Community Care Support Services to accomplish implementation goals.

The Clinical Coach will:
- Provide home care coaching, mentoring and supervision to home care staff and clinicians, to build primary-level home care competencies.
- Collaborate with First Nation home care leaders on local service integration, and change management initiatives, using quality improvement methodologies to achieve results.
- Consult on clinical care when the complexity of the clients' needs is beyond the existing competencies of care providers.

Required Skills:
WHAT YOU NEED TO BRING WITH YOU:
Education and Experience
- Registered Nurse (R.N) in good standing with the College of Nurses of Ontario, with substantive experience and training in home care.
- Minimum five (5) years' experience in the home care sector in Ontario is required.
- Knowledge of relevant legislation and reporting requirements, including the Fixing Long-Term Care Homes Act, the Excellent Care for All Act and the Compassionate Care Act and other pertinent legislation and regulations related to professional practice is required.
- Experience supervising clinical staff is required.
- Experience working with large-scale quality improvement initiatives.
- Additional training in gerontology and project management experience are assets.
- Strong clinical coaching and debriefing skills, with both individuals and teams.
- Strong consultation skills, with expert knowledge of models of consultation and their application with individual, team, and community service providers.
- Research and/or program evaluation experience with knowledge of evidence-based processes, including appraisal of research, application of findings and collaboration on research.
- Experience working with a range of roles, disciplines, and levels of staff in different health care settings.
- Experience working across health care settings.

Knowledge and skills
- A commitment to developing equity-based, evidence-informed approaches for delivering home care services for Indigenous clients, regardless of their age or illness.
- A good understanding of regional priorities and local home care needs, within Ontario's health care landscape.
- Ability to work well under pressure and use good judgment in assessing difficult situations.
- Ability to work independently and as part of a team.
- Aptitude for proactive problem solving, using strong critical thinking, and negotiating skills.
- A demonstrated ability to develop, deliver and evaluate training and coaching efforts.
- An ability to develop relationships based on trust, compassion, and empathy.
- Strong active listening and interpersonal communication skills for one-on-one coaching.
- Experience designing education to effectively address home care training needs of interprofessional learners.
- Commitment to engage with First Nations, Indigenous, Métis and urban Indigenous people as they self-determine their own approach to home and community care.
- Proficiency in educational technologies (e.g., webinars, apps) as well as knowledge of Microsoft Outlook, Word, Teams, PowerPoint, and Excel.
- Access to reliable and efficient mode(s) of transportation to enable travel between community organizations.
- A valid driver's licence and access to a reliable vehicle
- A clear vulnerable sector criminal reference check is a condition of employment.
Requirements:
WHAT IS EXPECTED? Given the traditional practices of Indigenous people, from time to time exposure to smoke from the burning of sacred medicines such as tobacco, sweet grass, sage, or cedar may occur.
