Position: Intake/Support Worker
Starting Range: Align with GRFN Paybands
Location: 23 Shingwauk Street, Garden River
Report to: Manager, Mental Health & Addictions
Duration: Full-Time

Overview
The Mental Health Intake & Support Worker will be a first point of contact for the MZW program and services. The position will provide support to the program through greeting, screening, receiving, processing, and tracking referrals, as well as applying knowledge of cultural, community and surrounding area resources to assist individuals seeking mental health supports. Collection of information, consent, data input and utilization of electronic medical records to open and close files and track referrals.
Administrative support such as scheduling, faxing, scanning, phone coverage, and delegation of other administrative tasks from MZW team. Identify and participate in community education and local/area promotional events.

DEPARTMENTAL DUTIES:
- Being the \"first point of contact\" for Garden River FN community members seeking mental health and addiction support via phone, in person and electronic communication
- Comfortable asking safety questions that assess whether an individual is at risk to harm themselves or others
- Accepting self referrals, internal referrals, and external/third party referrals via phone, email and fax
- Conduct initial screenings, ensuring referral meets eligibility criteria for service, assessing level of urgency, collecting missing information, following up with referral source as needed, and providing general support to open files as well as closing files
- Utilization of the electronic medical record system as well as other forms of electronic and physical organization tools to maintain consistent flow to referral responses
- Connect referrals to culturally relevant supports as requested/needed
- Work collaboratively with a multidisciplinary team to ensure clients are assigned to support staff based on initial intake and assessments
- Follow duty to report requirements as necessary
- Tracking referrals in order of priority and date of receipt in order to manage wait lists if and when necessary
- Responsible for opening files, closing files, updating information, scanning of documents into files, and requesting documents from external programs/services as needed
- Performing other related duties may be required as assigned

Required Skills:
Education and related experience around Mental Health & Addictions - Certificate or Diploma in Mental Health & Addictions,
Social Work or Health Services;
- Mental Health First Aid, Safe Talk, ASIST Training, Naloxone training considered an asset;
- 'G' Driver's License and clean driver's abstract;
- Experience using electronic medical records;
- Ability to use good judgement in assessing difficult situations;
- Flexibility required to keep pace with an ever-changing environment;
- Ability to be consistent and display a positive/helpful attitude;
- Ability to work independently and collaboratively as a member of a team;
- Excellent computer skills (including MS Word, Word Perfect, Excel, Internet, Email, Outlook);
- Ability to report for work as scheduled;
- Ability to attend work regularly;
- Must undergo a criminal record check prior to employment (At applicant expenses).
