Job Title: Adult Day Program Community Support Worker
Job Status: Part Time 21 hrs/week
Starting Wage: $21.00/hr. + group RRSP contribution
Job Location: Blind River
Description:
- Working as an integral member of the Alzheimer Society Adult Day Program the Adult Day Program Community Support Working will work 2 days supporting a variety of needs for clients attending the Adult Day Program in Blind River. The ideal candidate must be highly motivated, with a passion for persons living with dementia.
- The Adult Day Community Support Worker will provide a variety of activities in the community 1 day per week in Blind River and surrounding area.
- The Adult Day Program Community Support Worker will provide care in a safe and competent manner within their scope of practice.
Responsibilities:
1. Plan and organize various activities for clients in an Adult Day Program setting
2. Monitor participants throughout the day, assist, participate with bathroom routines when required and support mobility needs
3. Monitor and provide proactive follow-up for clients and care partners to ensure ongoing collaboration across services and identify opportunities for new or emerging care options to meet changing needs and to address service gaps.
4. Calmly and professionally react to crisis situations.
5. Provide activation to individual clients in community 1 day per week
6. Conduct programs with clients on a regular basis.
7. Provide support to co-workers, students and volunteers.
8. Evaluate the effectiveness of the programing for the clients.
9. Provide education and support to Persons Living with Dementia ad their families.
10. Act as a resource for inquiries from the general public regarding recreation/activation and Adult Day Program.
11. Demonstrated knowledge and vest practice in dementia, delirium, mental health disorders and their effect on persons, families, caregivers and the system of care.
12. Ensure that client consents, privacy and confidentiality are maintained in compliance with legislation, professional standards/regulations and internal policies.
13. Excellent communication (verbal and written).
14. Ability to take initiative and be resourceful.
15. Excellent problem solving and change management skills.
16. Adhere to program standards and Alzheimer Society policies and procedures.
17. Assume responsibility for personal upgrading in knowledge of Alzheimer's disease, and ongoing professional development.
18. Participate in fund raising and other team development activities.
19. Assume other functions as may be assigned b the Executive Director/ Team Lead.
20. Bilingualism (English/French) is an asset.

Required Skills:
Knowledge/Skills/Abilities:
1. Post-secondary degree or diploma in Recreation Therapy or graduate of an approved Personal Support worker program. Other health desciplines may be considered given appropriate combination of education and experience.
2. Knowledge of Alzheimer's disease and related dementias (ADRD)
3. Experience working with persons with dementia and their families.
4. Experience designing recreational programs to support individual needs as well as facilitating group activities
5. Knowledge of community health care agencies.
6. Current Standard First Aid/CPR certification.
7. Ability to work cooperatively with volunteers, staff, and community partners
8. Competency with Microsoft Office applications
9. Valid driver's license, regular access to a vehicle, and verification that the vehicle is insured for work purposes.
