Maamwesying North Shore Community Health Services is an Indigenous Health Care agency that provides primary health care, traditional healing, mental health and addictions, home and community support services to community members of the seven First Nations of the North Shore Tribal Council. We are inviting applications for a highly skills Nurse to join our Home and Community Support Services Team.

The objective is; to provide coverage for system navigation supports to ensure continuity of care for clients transitioning from hospital to home; and to provide comprehensive nursing care in the community setting that assists clients to achieve optimum health and quality of life in order to remain at home independently.

1.Registered Practical Nurse or Registered Nurse in good standing with the College of Nurses of Ontario
2.Indigenous cultural competency training certification
3.Minimum of 2 years nursing experience
4.Knowledge of system navigation, community resources, legislation, research and funding arrangements specific to Indigenous programs and services
5.Experience working with the elderly in a community setting is an assset
6.Ability to work with clients facing multiple barriers to health and well-being
7.Proven ability to apply advanced clinical nursing skills in a home care setting
8.Demonstrated ability to develop and facilitate health teachings
9.Possess a clear vulnerable sector criminal reference check is a condition of employment
10.Must be able to work flexible schedule to meet client needs, including evenings and weekends as required
11.Must have access to a reliable vehicle and a valid G Class driver's license
